If you have dental insurance you must realize that your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract. Our staff will be happy to discuss all insurance details with you at your appointment.
Our office accepts all Dental PPO Insurance Plans. We will accept assignment of benefits and file claims for your convenience. Please note that we are out of network.
A pre-determination can be submitted to your insurance company to determine your out of pocket expense before proceeding with treatment. Please allow 3-4 weeks for your insurance company to process the pre-determination.
At your request your treatment plan will be submitted to your insurance company before treatment to determine the amount the insurance company will pay. All co-payments will be due at the time of treatment. As a courtesy we will submit insurance for payment after treatment. If the insurance company defaults in full or partial payment, or fails to pay within a period of three months after treatment was provided, payment will be due from you at that time.
We will send you a monthly statement. Most insurances will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

All refunds will be provided as a credit to the credit card used at the time of purchase within five (5) business days upon receipt of the returned merchandise.
